Basic Hiring Package for Casual Employees
The following forms need to be completed and submitted to Human Resources for processing for a “casual (temporary ) employee.
- Personnel Action Form (A 3-part form which can be obtained from HR)
- Change of Address / Phone or Other Personal Information Form
- Completed Reference Check Form
- Computer Use Form
- Copy of Original Social Security Card (for payroll purposes)
- Drug Free Workplace Statement Form
- Health Questionnaire Form
- I-9 Form (Completed and signed, submitted with copies of valid identifications)
- Original employment Application (Completed and signed)
- W-4 From
The following may need to be completed by a select group of employees only. Check with Human Resources, Personnel Assistant at 459-1201 to determine if these forms need to be completed.
- Background Investigation Release Form
- Bloodborne Pathogen Information
- Bloodborne Pathogen Vaccination Form
Each new employee should receive a copy of the following documents. Print them, give them to the new casual employee and instruct them to read and ask questions.
